Application Materials
- Application Materials Checklist. This is a required component of all grant applications.
- Project Budget Template. This template is a required component of all applications for project grants; this form is not required for applications for general operating support.
Reporting Materials
- Financial Report Template. This template is required for all financial reporting on all project grants.
Foreign Grant Affidavit Materials
- Affidavit Form. If your organization is based outside the US, you may be required to include a foreign grant affidavit with your application. Please contact us before filling out the affidavit forms, as they are lengthy and time-consuming. We will review your organizational documents (bylaws, articles of incorporation, and equivalent materials) and let you know whether an affidavit will be necessary.
- Affidavit Support Schedules. If you are required to submit a foreign grant affidavit, you must submit these schedules as well.
